This is the most common Okin power supply for simple up and down chairs with 2 button hand controls and the two prong cable with two prongs, (One flat prong and one round prrong, pictured)  The original power supply was the  (3.00.207.036.00). Also number on old style is 129-N09A you should order with p120 cable

The two prong cable, our part #P120 cable that looks like this picture below plugs
into the end of the new power supply above and connects to a similar female pigtail on the motor, or
it may plug into a junction box.
